How can we ensure that home workers can continuously dispose of working tools?

Problem description

As we work with a shared caseload, and we can’t make use of a digital system during the home visits, we need an non digital alternative

Impact - effect

A lot of information is printed and destroyed, a lot of information is just forgotten at the office (e.g. psycho-education, crisis card, medication record….)

To do: develop a ‘to and fro map”, containing: address, telephone number, consent form, number of house visits, working tools
