How can we improve the communication between partners within the network?

Problem description

It’s not always clear which professionals are involved with a client.  Or which interventions are or were done by whom.  Who prescribes medication?  Which interventions were successful?

Impact - effect

Intake must be started up, again. Clients must bring their story, again. Clinicians miss information. Partners are not involved because of ignorance
