How can we still improve our daily practice by using practical and targeted tools?

Problem description

As a young 2B-team we are still looking for tools to support our daily practice, and this in various areas.   Especially regarding the contribution of user himself and the network around him, but also regarding the pro-activity (pre-phase) of an intervention by our team and a systematised support (inter- and supervision system) that can allow the team to view situations differently.

Impact - effect

The tendency to project your own vision on the user is still present.   Hospitals are often thinking too late about an orientation to the mobile team.  Contacts between different services remain relatively limited

To do

To do: explore and discuss the potential added value of working with instruments like those used in Lausanne: Eladeb (self-evaluation by the user), network map (how the patient is looking to his own network), participate at the meetings of partner services, supervision (critical view at the own practice).
