How to guarantee a recovery project?
Problem description
One of the first requests for the mobile team at the beginning of the care taking seems to be a form of relay to "something else." A relay to broad areas such as nursing, recreation, therapy, help with daily living
Impact - effect
These types of relay rarely sustain. The rehabilitation provision does not seem to be adequate in relation to the many and varied demands. This is resulting in an inadequate follow-up by the mobile team in the timeline of the person
