There is no intensive contact between the hospital and the mobile team when a client of the team is admitted to hospital

The team is informed afterwards, so it’s only after a few days that objectives and timings can be discussed
To do
To do: a team member is present at the decision making process leading to an admission, and if this is not possible, a care coordination meeting must be organised within 24 hours. In this way it more likely that the client will probably understand the admission
