How, in a crisis situation, improve the maintaining in the home situation by activating all possible resources?

Problem description

Interventions based on the strict model of “colloque singulier” (one-to-one relation patient and usually a doctor, the doctor-patient bond where reference is made to confidentiality, secrecy and discretion) are often inadequate to maintain treatment in the home situation

Impact - effect

When the upper limit of intensity of interventions by the mobile teams is reached (limits of frequency, of duration), a hospital admission is often inevitable when no other resources are available

To do

To do:  maintaining the home situation as treatment location is facilitated, even in a period of crisis, by a proactive involvement of the environment, and other professional resources  (who are informed immediately and systematically by a specific form called ‘medical correspondents”)
