How can we ensure continuity of care in a crisis situation?

Problem description

Working with half-time employed team members, plury-disciplinary, and limited in volume (limited number WTE), means there is no guarantee the one and the same team member  - sometimes called the allocated key worker – is able to be responsible for the follow up

Impact - effect

Given the short term interventions in the context of a crisis, a lack of continuity of care must absolutely be avoided.  The nature of the crisis must allow that all aspects can be brought into account

To do: from the moment of demand, every situation is analysed by at least two members of the team and recorded using a standard form. In addition, each team member is expected that he or she may intervene in any situation of persons belonging to the active caseload, at least in pairs including a doctor who also works in a mobile way
