How to simplify the exchange of information, how to  make this more accessible and rapidly available for every professional ?

Problem description

It’s not easy to contact professionals (GP, psychiatrist, social workers of several services,..), because of the agendas, work schedules, meetings, etc.

Impact - effect

Delay in the exchange of information, in updating,…  Losing time in monitoring the patient, spend more time in tasks where others have already invested some time.  Delaying intervention, which is frustrating for the user and professional
