What are possible handles for short term interventions that make it possible to establish a therapeutic relation enabling continuity of care?
Problem description
If one adheres to brief interventions solely focussing on the crisis, this can lead to the absence of a relay for care activities following the crisis period. It’s not clear how interventions can be limited in time, and yet ensuring continuity of care
Impact - effect
our teams do not make a real choice, various visions continue to coexist, there is often a tendency or habit to intervene longer and thoroughly, and the wish to remain involved as a professional in taking decisions about follow-up care. By consequence different expectations persist with team members and network partners on the tasks, objectives and methodology of a 2a-team (for whom? For how long, etc.)
