How can we improve care transfer to professionals continuing the pathway?

Problem description

At short notice we try to create a strong relationship of trust.  For patients, a transfer of care is often experienced as ‘back to zero, again’

Impact - effect

Patients don’t engage with the proposed continuing care, they drop out

To do

- Organise multidisciplinary consultations

- Organise joint visits (see HTT Oleaster, Birmingham)

- Prepare patients in a better way for the next step, more emphasis on the 2a limited intervention period
